Special Initiative: Capturing Lessons Learned from the Field: Healthy Start Town Hall Webinar

This town hall meeting will describe a strategy to engage Healthy Start programs to share their lessons learned and promising and best practices in implementing Healthy Start 3.0 and provide an overview of the process and timeline for engaging Healthy Start programs to provide their perspectives from the field.

As a follow-up to the May 16th, Capturing Lessons Learned from the Field: Healthy Start Town Hall, we are posting a link to the lessons learned input survey.  The purpose of this survey is to provide Healthy Start programs the opportunity to provide perspectives from the field by sharing promising and best practices they used to implement Healthy Start 3.0 and lessons learned from those experiences.

Process for Submission of Feedback: 

Please review the pdf version of the Healthy Start the lessons learned input form that includes specific areas to provide feedback with all staff and key stakeholders from your HS site who serves participants. After reviewing the Feedback Summary Form, your site should work as a team to summarize and submit your organization’s feedback. We will be accepting one feedback form per Healthy Start program.

This information will be shared with the Division of Healthy Start and Perinatal Services in order to shape future programming.  The information you provide to the survey will not be aligned in any way to you individually or to your Healthy Start program; a summary of the survey data will be was developed in order to eliminate the potential for specific responses to be linked to any particular individual HS program.  Your information will be analyzed in aggregated form and reported in a summary format.
