The ICP provides clinical care, care coordination, and health education to underserved or uninsured women with prior poor birth outcomes. Objectives include improving the health of women prior to pregnancy, increasing the number of women who deliver at term, improving birth intervals, increasing access to quality preconception, interconception, prenatal, and postnatal health care, and providing continuity of care and care coordination from the postnatal period through subsequent pregnancies.
